Dr. Malin received his D.D.S. degree from Marquette University in 1991. He maintains a private practice in Onalaska, Wisconsin. Dr. Malin’s dental practice is exclusively limited to the surgical and restorative phases of Implant Dentistry. Dr Malin is also the Director of the Implantology and Bone Grafting Courses at the Las Vegas Institute for Advanced Dental Studies (LVI). Dr. Malin is the instructor of a variety of courses each year, ranging from implant basics to advanced surgical and restorative protocols. Doctors across North America and abroad attend these courses.
Early in his career, Dr. Malin discovered that the implant placement techniques typically used would not deliver a standard, predictable result. Because of this Dr. Malin, along with other experts in the fields of radiology and occlusion, has developed an occlusion based implant placement technique. This technique integrates the latest technologies to guarantee implant case control. This technique has also led to four United States patents and provided exceptional clinical implant surgical and restorative results. Dr. Malin additionally lectures at venues across North America on full mouth reconstructions and implant placements. He has received a variety of awards from his contributions in implant and neuromuscular dentistry.

Dr. Alonge is a native of the Erie, Pennsylvania region. He attended Allegheny College where he graduated with a Bachelor of Science degree in Biology and a minor in English (1977). Dr. Alonge was accepted into the Graduate School of the University of Maryland and while working towards his master’s degree in Anatomy he was accepted to the Baltimore College of Dental Surgery, Dental School, University of Maryland at Baltimore – the world’s first dental school. While in dental school he was elected class president each of his four years and concurrently completed his Master of Science degree.Dr. Alonge graduated Magna cum Laude from dental school in 1983 and received numerous awards and honors. Among them, election to Phi Kappa Phi National Honor Society for graduating in the top five percent of the entire University of Maryland system; Omicron Kappa Upsilon, the national dental honor society; the Timothy O. Heatwole Chair to the senior who has best exemplified the qualities of ethical standards, kindness and humanitarianism and the Harry E. Kelsey Award for Professional Demeanor.
Dr. Alonge was selected for the United States Air Force General Practice Residency Program at Davis- Monthan AFB in Tucson, Arizona which he completed in 1984. He was then transferred to Hickam AFB, Hawaii where for three years he served as Chief of Outpatient Oral Surgery prior to entering formal Oral and Maxillofacial Residency Training at Wilford Hall USAF Medical Center in San Antonio, Texas. Upon completion of his surgery training in 1990, Dr. Alonge was then assigned to Wright-Patterson USAF Medical Center in Dayton, Ohio where he served as Oral Surgery Training Officer for the Advanced Education in General Dentistry Residency Program. He separated from the Air Force in 1994 to enter private practice in Erie. Dr. Alonge is board certified in his specialty and is the first in his region to successfully recertify with the American Board of Oral and Maxillofacial Surgery.
As a result of his years of teaching, Dr. Alonge has extensive experience in all subspecialty areas such as dentoalveolar surgery, dental implants, corrective jaw and facial surgery. Dr. Alonge is especially talented in management of anxious patients and is the first oral surgeon in his region to be certified by the National Dental Board of Anesthesiology.
Dr. Alonge has been certified in Advanced Trauma Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support and serves his local peers as a Basic Life Support Instructor. He is the only dentist from his local dental society to be elected President twice. He and his wife have two children who are currently in college.

To sit for the examination, Dr. Reese was required to practice implant dentistry in Indianapolis for at least six years, and prove his implant expertise. To earn the title of Diplomate, Dr. Reese presented cases for review by an expert panel of implant dentists and passed a series of written and oral examinations sponsored by the American Board of Oral Implantology/Implant Dentistry.
Additionally Dr. Reese takes part in medical-dental outreaches to El Salvador, Central America, Haiti, and Africa with the intent to help improve the quality of life for those who are impoverished and suffering.
Dr. Rapoport attended the University of the Witwatersrand in Johannesburg and after graduating in 1990, practiced general dentistry for 6 years. In 1996, Darrin and his wife Pam relocated to Seattle where he received his Masters of Science in Dentistry from the University of Washington and completed 3 years of highly specialized periodontal and implant training.
Dr. Rapoport is licensed to practice in Washington State and internationally, in South Africa, the United Kingdom and Australia. He is a board certified Periodontist and Diplomate of the American Board of Periodontology, the highest honor awarded in his field. He is an active member of multiple organizations including the Seattle-King County Dental Society, the Washington State Society of Periodontists, the American Dental Association, the American Academy of Periodontology, the International Congress of Oral Implantologists and the Academy of Osseointegration. In addition, he regularly lectures at national and international events and has been published multiple times.
In 2001 Dr. Rapoport joined Pacific Northwest Periodontics and purchased the practice two years later. He and his team are committed to providing exceptional periodontal and implant dentistry care. Dr. Rapoport is regarded as a local dental expert, contributing to several KOMO NewsradioLifebeat segments and appearing on KOMO 4, KING 5's Health Link, and KCPQ 13. Dr. Shamardi trained for his Periodontal certificate at the prestigious University of Pennsylvania Periodontal Program. During this training, he had extensive nonsurgical and surgical experience for procedures such as sinus lifts, implants, mucogingival surgery, crown lengthening, osseous and guided bone and tissue regeneration. Additionally, the exposure to the unique Periodontal Prosthesis program has allowed him to better understand the restorative aspect of the profession and the importance of proper treatement planning. Having graduated in 2010, Dr. Shamardi returned to California, spending one year working in multiple practices and becoming a Diplomate of the American Board of Periodontology, the heights honor in his field. He then returned to Boston in July 2011 and has since been working full time and teaching part time as a Clinical Instructor at Harvard school of Dental Medicine, Division of Periodontics.
Dr Hunt is a fourth generation dentist trained originally in Great Britain at Guy’s Hospital Dental School in the University of London. After graduation he spent a year in residencies in Maxillo-Facial surgery before obtaining a Master’s in Advanced Restorative Dentistry at the Eastman Dental Center in London. He then received a Scholarship to the University of Pennsylvania where he obtained Specialty certificates in Periodontics and Periodontal Prosthesis.
Mark A. Iacobelli DDS, FAGD, FICD, MIIF, is a graduate of Case Western Reserve University School of Dentistry. Since his graduation in 1982, he has maintained a full time, fee for service private practice in suburban Cleveland, Ohio.
Dr. Robert D'Orazio is a dentist at the Dental Implant Institute in Sterling Heights, Michigan. He specializes in dental implants, cosmetic dentistry, periodontics, and endodontics. Dr. D'Orazio is a 1984 graduate of the University of Detroit, School of Dentistry. In 1987, he obtained a Fellowship in the Academy of General Dentistry. In 1991, Dr. D'Orazio completed a two year externship at the Midwest Implant Institute, which included obtaining an ACLS certificate and intravenous conscious sedation training. Dr. Gelb completed his DDS and Certificate in Periodontics at Columbia University School of Dental and Oral Surgery in New York and his implant and surgical training at the University of Toronto. He is a Diplomat of the American Academy of Periodontology, a Fellow Academy of Osseointegration, and a Fellow American College of Dentists. He is on staff at Hartford Hospital and has provided implant services in West Hartford for twenty years. Dr. Gelb has achieved global recognition as a leader in Implant dentistry having pioneered surgical procedures for finite esthetics in implant dentistry, immediate placement, and bone regeneration. He lectures nationally and internationally at major implant meetings and publishes extensively in journals worldwide on implant dentistry. He developed the "Gelb radiographic depth gauge," a diagnostic surgical guide to heighten precise and optimal implant placement, and designed "The Implant Tracker," a comprehensive software program to manage the Implant practice. Dr. Gelb maintains a private practice limited to Periodontics and Implant Surgery in his state-of-the-art facility in West Hartford, CT, where he and his dedicated staff serve patients from all over the world.
Dr. Weiss is a Diplomate of the American Board of Periodontology, a Fellow of the American College of Dentists and a Fellow of The International Team for Implantology, and has been practicing periodontics and implant dentistry since 1992. He received both his D.M.D. and B.A. in biology from the University of Pennsylvania, and completed a residency in periodontics at Temple University. Additionally, Dr. Weiss is a past president of the Hudson County Dental Society, and an attending periodontist in the Department of Dentistry, Division of Periodontics at Newark Beth Israel Medical Center. He is also a Clinical Assistant Professor at The University of Medicine and Dentistry of New Jersey and at The University of Pennsylvania School of Dental Medicine. Dr. Weiss lectures extensively on many topics related to periodontics and implant dentistry.
Clinicians and researchers have developed recommendations regarding implant position, dimensions and numbers but the area of surgical technique and instrumentation to preserve native bone has been under-emphasized.
Dr. Henry W. Ferguson received his dental degree in 1980 from The University of Pittsburgh School of Dental Medicine, Pittsburgh PA. Upon completion of Dental School, Dr. Ferguson entered into active duty service with the United States Army Dental Corps.
After seven years of commendable service as a general dentist, Dr. Ferguson was selected for training in Oral and Maxillofacial Surgery (OMS). He completed a residency program in Oral and Maxillofacial Surgery at the Eisenhower Army Medical Center, Fort Gordon GA., in 1991.
After years of distinguished and meritorious service, Dr. Ferguson retired from active duty at the rank of Colonel in March 2004. His last assignments in the Army were as a staff OMS and member of the teaching faculty with the Department of Oral and Maxillofacial Surgery, Eisenhower Army Medical Center. Additional assignments were as the Department Chairman and Director of OMS Residency Training, and Director of Hospital Dentistry, Eisenhower Army Medical Center, Fort Gordon, GA.
In April 2004, Dr Ferguson received a joint appointment as an Associate Professor in the Medical College of Georgia Medical Center Department of Surgery, and the Medical College of Georgia School of Dentistry as an Associate Professor in the Department of Oral and Maxillofacial Surgery. He currently serves as the Vice Chair, and Director of Post Graduate training for the Augusta University (formerly the Medical College of Georgia), Dental College of Georgia, Department of Oral and Maxillofacial Surgery.
Dr. Ferguson is a Diplomate of the American Board of Oral and Maxillofacial Surgery, member of the ADA, a fellow of the American Association of Oral and Maxillofacial Surgery, the National Society of Oral and Maxillofacial Surgeons, the American Dental Society of Anesthesiology, and Diplomate of the National Dental Board of Anesthesiology.
Dr. Joseph A. Favia is a 1993 final graduate of Loyola University School of Dentistry where he served as it's final Student Congress President. He has taken advanaced dental education from some of dentistry's best institutions including The Peter Dawson Academy for Advanced Dental Education and The Pankey Institute. Dr. Favia has been teaching for the two largest dental supply companies since 2005. He taught surgical placement for Henry Schein to general dentists from all over the US and Canada and has founded The Midwest Surgical Institute of Dental Implants (MSDI) after many requests from his doctors to learn advanced procedures in 2009. Dr. Favia completed an advanced bone grafting mentorship with Dr. Joel Rosenlicht in 2005. In 1997, Dr. Favia founded and chaired for over 14 years The Windy City Classic Celebrity Golf Tourney which has raised over 1 million dollars toward cancer research in memory of Jim Valvano and Walter Payton. Dr. Favia served as 2010 President of the Arlington Heights Chamber of Commerce where he founded the very first Taste of Arlington Heights, an event he still chairs today. He was honored as the Emerging Business of the Year in 2005 and Business Leader of the Year in 2011 from The Arlington Heights Chamber of Commerce. Dr. Favia received the Daily Herald's (Chicagoland's 3rd largest newspaper) Reader's Choice Award as "Best Dentist" in 2013. Dr. Joe lives and maintains a private practice in Arlington Heights, IL. with his wife, Julie, and three children.
